Insurance Information


We accept insurance for all medical, non-cosmetic visits.  If your insurance requires a referral, it is your responsibility to provide one prior to or at the time of service. If you do not have a referral, you will be required to pay in full or reschedule your appointment. We participate with numerous insurance companies; however we do not participate with the following:


  • Any form of Medicaid  
  • CCC Plus Plans
  • Dual Medicare/Medicaid Plans.  
  • United Health Medicare Advantage plans
  • Anthem Healthkeeper Plans
  • Tricare Prime – without a referral
  • Any form of Car Insurance
  • Workers Comp without a Valid Referral
  • Cigna True Choice Medicare Plans



We are often asked if we accept insurance for certain cosmetic procedures. The majority of elective aesthetic procedures are NOT covered by insurance. However, some aesthetic treatments are also medical in nature (for example, drooping eyelids that impede one’s vision, excessively large breasts that cause documented back pain). In some cases, medical insurance may be accepted, depending on the insurance carrier. Please discuss this with Dr. Asfa during your consultation.
