Appointments

All consultations with Dr. Asfa are by appointment. After you have made your appointment, you will be emailed a link to our confidential, HIPPA-compliant patient portal so you can complete any required medical forms (if you prefer we can mail the forms to you). We strive to see all of our patients on time, and having your forms completed in advance assists us with staying on schedule. 

Cosmetic Consultations

For elective cosmetic or laser consultations, a credit card number must be provided to reserve your appointment (non-refundable fee of $90 for cosmetic/$50 for laser). This fee is deducted from the surgery price when booked, or injectables performed the same day as the consultation. 

Cancellations

We make every effort to remind our patients about scheduled appointments via your preferred contact method. As a courtesy, please provide a minimum of 24 hours notice for appointment cancellations. Failure to cancel your appointment will result in a $90 “No Show” fee, which is subject to collections if not paid in a timely manner. Failure to keep appointments without notifying our office interferes with your healthcare, as well as our ability to accommodate other patients waiting for appointments.

Late Arrivals

Dr. Asfa is often fully booked, and we make every effort to stay on schedule. In fairness to all of our patients, if you arrive more than 15 minutes late for your appointment, your consultation time may need to be reduced or rescheduled. Please call us if you are running late.

Inclement Weather

There may be times when we need to close the office, operate on a delay or close early due to inclement weather conditions. If/when this happens we will make every effort to contact anyone who has an appointment on the affected day(s). If you do not receive our messages, you may listen o our outgoing phone message or check our social media pages for updates.

Insurance

We accept insurance for all medical, non-cosmetic visits.  If your insurance requires a referral, it is your responsibility to provide one prior to or at the time of service. If you do not have a referral, you will be required to pay in full or reschedule your appointment. We participate with numerous insurance companies; however we do not participate with the following:

  • Any form of Medicaid  
  • CCC Plus Plans
  • Dual Medicare/Medicaid Plans.  
  • United Health Medicare Advantage plans
  • Anthem Healthkeeper Plans
  • Tricare Prime – without a referral
  • Any form of Car Insurance
  • Workers Comp without a Valid Referral
  • Cigna True Choice Medicare Plans

We are often asked if we accept insurance for certain cosmetic procedures. The majority of elective aesthetic procedures are NOT covered by insurance. However, some aesthetic treatments are also medical in nature (for example, drooping eyelids that impede one’s vision, excessively large breasts that cause documented back pain). In some cases, medical insurance may be accepted, depending on the insurance carrier. Please discuss this with Dr. Asfa during your consultation.

Financing

Get the look you want now and pay later! We work with several lending companies that allow our patients to pay in installments for surgical and non-surgical cosmetic procedures, injectables, skincare products, laser treatments and more. The companies we work with are CareCredit, Cherry, and PatientFi, and you may apply via their websites.
